Form -IV A & B - Annual return of payment & wages.(Rs 3 allowance/day in spraying)


Form - C- Assam tea employes welfare vfund act.

ER-1 - Monthly central excise factory report.


ER -5 -Monthly green leaves report.


GAR-7 - Monthly excise duty return.


Form -G - Meternity benefits Register.


Form -H -Annual meternity benefit Act.


Form -I - Application for gratuity by employee.


Form -J - Application for gratuity by Nominee.


Form -L- Notice for payment of gratuity & its calculation.

(Gratuity Calculation =Wages/day X 15 (for 1year) x Years of Service.)


Form - MM - Monthly monitoring of gratuity & P.F. settlement.


W-03 - Firewood issued Register.

(Firewood to be issued - @1 stake (228 cft)/ worker
 & 
Ration @ 3.26 kgs/ worker/week and deducted amount @54 Paisa/kg. 
Black Tea @ 0.9 kg/ month/ worker)
